Every school reserves the right to admit or refuse any student applicant, seeking entrance in the school. For our school, it is only when the parents/guardians have agreed to subscribe to the Philosophy, Vision-Mission and Objectives of the NSDA Parochial School , the Ideals of Catholic Education, and to comply with the Scholastic Standards of the School, as well as to observe all its rules and regulations that a student applicant can be admitted.

ADMISSION REQUIREMENTS


•  PRE-ELEMENTARY DEPARTMENT

A.1 Age Requirement :

A.1.1. NURSERY - 3 ½ - 4 yrs. old at the opening of the school year

A.1.2. KINDERGARTEN – 4 ½ - 5 yrs. old at the opening of the school year

A.1.3. PREPARATORY – 5 ½ - 6 yrs. old at the opening of the school year

 

B. Other Requirements :

B.1. Birth and Catholic Baptismal Certificates (original & photocopy)

B.2. Entrance test fee of Php100.00

B.3. Oral Entrance test for Nursery

B.4. Written Entrance test for Kinder & Prep

B.5. Interview with the Principal or her representative.

B.6. Reservation fee of Php500.00

 

•  ELEMENTARY DEPARTMENT

A. Age Requirement:

A.1. Grade 1 – 6 to 7 yrs.old at the opening of the school year

With Pre-elementary preparation

 

B. Other Requirements: ( from Gr.1 to 6)

B.1. Birth and Catholic Baptismal Certificates (original & photocopy)

B.2. Entrance test fee of Php100.00

B.3. Certification of Good Moral Character from previous School Principal

B.4. Original & Photocopy of Report Card

B.5. Written Entrance test

B.6. Interview with the Principal or her representative

B.7. Reservation fee of Php500.00

B.8. Form 137

 

•  HIGH SCHOOL DEPARTMENT

1. Birth and Catholic Baptismal Certificates (original & photocopy)

2. Entrance test fee of Php100.00

3. Letter of Recommendation from previous School Principal

4. Original & Photocopy of Report Card

5. Written Entrance test

6. Interview with the Principal or her representative

7. Reservation fee of Php500.00

8. Form 137

D. TRANSFEREES

Report card (Form 138-E) from the school last attended.
Letter of Recommendation from the school last attended.
Certificate of Good Moral Character
Birth and Baptismal Certificates ( original and photocopy )
Entrance fee

1. Student should pass the School Entrance Examination.
2. Student should pass the interviewed done by the school Principal or an authorized personnel.

 

ENROLLMENT PROCEDURE:

(Note: Old students whose Report Cards were withheld must accomplish their clearance slip first before they can proceed with the enrollment process. )

Old students who are on Academic and Disciplinary Probation must see first the Student Affairs Coordinator (OSA) and the Prefect of Discipline (OPD) before they can proceed with the enrollment process).

Step 1. Registrar's Office:

Present the student Report Card for cancellation. Old students on Academic/Disciplinary Probation must present a clearance slip from OSA & OPD. Secure a registration form and fill up properly.

Step 2. Assessment Clerk:

Present the filled up registration form and wait for the Assessment of fees.

Step 3. Cashier:

Pay pertinent fees at the cashier only. A student is considered enrolled only after payment of required fees.

Step 4. Supplies, Uniform & Bookstore:

Present the receipt of enrolment

Assigning of class sectioning and sale of grade or year level patches will be given on the 3 rd week of May.

 


Admission Alma Mater

ADMISSION POLICIES AND COMMITTEEAdmission is a very important task of the school. Thus, policies on admission, retention, and transfer of students have been formulated.An Admission Committee is organized and tasked with the following responsibilities:

To handle all matters related to admission according to the stated standards, policies and objectives of the school as a whole and the department in particular ;

To evaluate the standards, norms and procedures on admission and to coordinate with the Guidance Office on the proper placement of students;

To recommend for the proper and efficient registration of students;

To help formulate definite policies regarding the academic probation and withdrawal of students;

To decide on all irregular applications.
